Permanent CPS Registration

  1. Application form duly in and signed (at one place) by the applicant only.
  2. The applicants should remember that their names entered in the application must exactly correspond with their names at the university or other examination as the case may be.
  3. All the original certificates should be enclosed along with the Xerox copies, otherwise original certificates will be retained in the office of the council. Original documents will be returned back after verification of application.
  4. CPS Provisional Registration Certificate issued by Gujarat Medical Council (Original).
  5. Gujarat Medical Council Permanent Registration Certificate. (Xerox)
  6. CPS Permanent Degree Certificate. (Original + Xerox)
  7. Bonafied Certificate issued by institute. (Original + Xerox)
  8. Government of Gujarat rural completion certificate. (Original + Xerox)
  9. Maharashtra Medical Council NOC & Registration Certificate for CPS Qualification. (Original + Xerox)
  10. Registration certificate will be issued after one month from the date of submitting the application form & if application is not kept in pending for want of any above certificate etc.
  11. Form of on behalf be filled in, if applicant has not submitted his application in the office personally.
  12. Registration fee Rs. 10,000/- by only Demand draft in favour of “Registrar Gujarat Medical Council” payable at ahmedabad preferable of nationalized bank only, and Rs. 100/- may be remitted extra, if you desire to get your registration certificate by registered post and Rs. 500/- for online charges.
