Passed from Gujarat State Institutions

  1. Application should apply on line at www.gmconline.co.in , Application Form duly filled in and Signed (at three places) by the Applicant only.
  2. Registration Fee of Rs.1000/- (Rupees One Thousand Only.), Administration Fees Rs. 500/- (Five Hundred only) by only Demand Draft in Favour of Registrar, Gujarat Medical Council, Payable at Ahmedabad preferable of State Bank of India. (And Rs.100/- (Rupees One Hundred Only.) may be remitted extra, if you desire to get your Registration Certificate by Registered Post.) Candidate has to pay total fees Rs. 1600/- By Only Demand Draft in Favour of “Registrar, Gujarat Medical Council”, Payable at Ahmedabad. Preferable of State Bank of India or Any Nationalised Banks.
  3. The Applicants should remember that their names entered in the application must exactly correspond with their names at the University or other Examination as the case may be.
  4. All the ORIGINAL CERTIFICATES should be enclosed along with the Xerox copies, otherwise original certificates will be retained in the office of the Council. Original documents will be returned back after verification of application.
  5. Internship Completion Certificate granted by the University (Passing Certificate) in ORIGINAL with One XEROX Copy.
  6. 12 Months Internship Completion Certificate issued by the Dean of Medical College in ORIGINAL with One XEROX Copy
  7. Provisional Registration Certificate issued by this Council in ORIGINAL. XEROX COPY IS NOT REQUIRED.
  8. School Leaving Certificate / Birth Date Certificate / S.S.C.E. Certificate (For Birth Date) in ORIGINAL with ONE XEROX Copy.
  9. M.B.B.S. Degree Certificate (Convocation Certificate) issued by concerned University in ORIGINAL with ONE XEROX Copy. (If available).
  10. Evidence regarding change of Name, Surname be sent viz Gazette / Marriage Registration Certificate as the case may be.
  11. Registration Certificate will be issued after One Month from the date of submitting the application form & if application is not kept in pending for want of any above Certificate etc.
  12. Form of On Behalf be filled in, if applicant has not submitted his application in the Office Personally.
  13. Affidavit in the following Format on Non-Judicial Stamp Paper of Rs.10/- duly sworn in and Attest by First Class Magistrate. For delay in Applying for Registration in case delay is more than SIX MONTHS after completion of Internship Training.
  14. Three Colour Passport Size Photographs duly signed by applicant doctor on front side of the photographs.  

Passed from Out Side Gujarat State Institutions

  5. All Final M.B.B.S. Mark sheets (Part-I & Part-II) granted by the University including Fail Mark sheets in ORIGINAL with One XEROX Copy.
  6. University Completion / Provisional Degree Certificate granted by the University in ORIGINAL with One XEROX Copy.
  7. M.B.B.S. attempt Certificate issued by the College in ORIGINAL with One XEROX Copy.
  8. M.B.B.S. Degree Certificate i.e. Convocation Certificate be also produce at the time of submitting Application Form. In ORIGINAL with ONE XEROX Copy.
  9. Concerned State Medical Council’s Registration Certificate in ORIGINAL with One XEROX Copy.
  10. No Objection Certificate (NOC) is required to be produce from the Concerned State Medical Council with which Registered along with Application Form.
  11. Form of On Behalf be filled in, if applicant has not submitted his application in the Office Personally.
  12. Three Colour Passport Size Photographs duly signed by applicant doctor on front side of the photographs.

Passed from Out Side India Institutions

  1. Application should apply on line at www.gmconline.co.in , Application Form duly filled in and Signed (at three places) by the Applicant only.
  2. Registration Fee of Rs.7,000/- (Rupees Seven Thousand Only.), Online registration Fees Rs. 500/- (Five Hundred only) by only Demand Draft in Favour of Registrar, Gujarat Medical Council, Payable at Ahmedabad preferable of State Bank of India. (And Rs.100/- (Rupees One Hundred Only.) may be remitted extra, if you desire to get your Registration Certificate by Registered Post.) Candidate has to pay total fees Rs. 7500/- By Only Demand Draft in Favour of “Registrar, Gujarat Medical Council”, Payable at Ahmedabad. Preferable of State Bank of India or Any Nationalised Banks.
  3. Any Certificate i.e. School Leaving, Birth Date or S.S.C. Certificate containing your Birth Date in ORIGINAL With ONE XEROX Copy.
  4. All Documents which you have passed from outside of India in Country’s language and English Version.
  5. 12 Months Internship Completion Certificate issued by the Dean in ORIGINAL with One XEROX Copy.
  6. MCI Screening Test Result in ORIGINAL with One XEROX Copy.
  7. MCI Registration Certificate in ORIGINAL with One XEROX Copy.
  8. No Objection Certificate (NOC) is required to be produce from the Concerned State Medical Council with which registered along with Application Form.
